I know you've got a ton of replies already, but I must say that I personally believe that blow-out is a myth. I was talking to Benson Bell (an extremely well-known bassoon makes) who has repaired many so-called "blown-out" instruments. He believes that it's from the keys continuously pushing down on the top of the tone-hole, which gets worn down with time. He builds them up to what they were and the instrument works almost as good as new. He has not done it to my oboe, but I have watched him fix enough instruments to have complete faith in what he says.
