Sorry about joining this string late (I haven't been here for a while), but I just noticed something in Dutchy's original post that might be important.
Dutchy mentioned SLIDING the left index finger to operate the half hole. An alternate way is to ROLL your finger on and off the half hole by rotating your finger and your wrist. I always thought this was the best way, especially for those of us with ridiculously tiny pinky fingers. It works with English horn and d'amore as well, but a bit more rolling is needed to operate the split plate. The general rule still applies: comfortable hand position with minimal finger movement and snappy finger action.
