Arthritis has made bearing the weight of oboe on right thumb a problem involving the wrist and thumb. Neck strap is no help, but a gadget called the Fhred, Quodlibit Inc, Englewood, CO. has helped considerably. I still prefer the freedom of motion (especially with my need for trifocal eyeglasses) and can play short periods without . But for long rehearsals etc. the Fhred is very good. I needed one two years ago for the heavier English Horn but the angle when playing EH is easier than with oboe. The newer gadget involving supporting the bell on stand may be better but is considerably more expensive. The telescoping Fhred fits neatly into your gig bag or zippered case cover.
