SEE A PHYSIOTHERAPIST! Try to find one with experience treating musicians specifically.
Doctors are clueless in such cases. Strengthening can actually cause more harm if there is an underlying problem.
The right hand has one important hindrance: carrying the weight of the instrument. As you know, the oboe has ZERO benefit from neck straps. I used to tie my bell via hook to my music stand: MUCH more beneficial. You can also find links to special thumb-rests and bell supports (really good one) on my blog:
