Make sure you have enough reed in your mouth, and it is really imperative that you are not squeezing the reed. Your embouchure needs to be relaxed (yet firm enough to support the reed), and you should have a clear picture in your mind of a beautiful, round low D sound. (It sounds corny, but visualizing the sound you want and the correct pitch is VERY helpful in actually producing them).
Much like the flute, when you're playing downward octaves, you should be aiming for the low note, and the higher note should be almost a secondary mental afterthought.
