I had the same problem; sling did not help. Didn't matter if my right thumb was even touching the instrument. It was not a weight issue, because making a stand that took 100% of weight did not help either.
It was a hand size issue. I solved it by getting a custom, more-curved bocal made by Dallas. My teacher (who is high level pro) says it is a good bocal, despite being more curved. It certainly seems to me to play as well.
With the more-curved bocal, not only is the instrument raised about a half inch with respect to my face, it also is in closer to my body, which supplies the other half inch of reach I needed. I still use the home-made stand to take the weight. We made that out of a $1 wood counter-standing paper towel holder from a thrift shop, some PVC, a couple of cotter pins, and a hook that I rest the bell on. With the mods we made (with a saw and a drill,) it is height-adjustable and, at $1 and some fiddling, *immensely* cheaper than something like a Phred, even though maybe not as pretty. Not being rich, I'll take it. It even comes apart to fit in my bag.
