Due to arthritis in my right hand, I have extended the F# as suggested, moved thumbrest upward, and also use a Fhred support when playing the English Horn. Otherwise it would be impossible for me to use the low C# as you described and also the left hand E flat . As the arthritis deformed my fingers more and more each of the above helped and I can still play. The Fhred really helps a lot, a neckstrap also helped but the Fhred allows more freedom of motion and doesn't make my neck tired. Lois Barton
