I think any performer has way too many stories to tell about this sort of thing...my teacher had a great one last year. He was in the middle of a concert (playing principal oboe) and started thinking about how he had forgotten to bring a reed knife to give to the conductor to give to a former student, who played in the conductor's orchestra. He was quite absorbed by this, and looked up to notice that the conductor was staring at him quite strangely. He had become so wrapped up in this knife thing that he had forgotten to play a big solo!
The audience may have never known, but everyone around him sure did...
