remember the low C pinky key has to be down to voice Hi D easily, and both octave keys must be shut, when playing after C" it's easy to miss applying low C and temptingly easy not to let go of the 2nd 8ve
... other things to consider
--where exactly your lips are on that particular reed, and what pressure adjustments you might be making, unconsciously in particular, and does another reed gives you the same trouble or not?
-- if it does, then check the oboe for bunged half-hole or perhaps a wiggly screw in the semi-auto 8ve mechanism so it isn't shutting properly even when you've let go of it
