Top D not working properly

The top D (D") on my oboe is doing peculiar things at the moment. Instead of the note sounding immediately there is a low noise (I hestitate to call it a note) a sort of almost inaudible pop and then the D comes out. If I use a huge amount of breath pressure (much more than I would usually and enough to make any other note squeak) then it will come out straight away. Having got it to come out I can slur up and down to it from different notes but if I play more than a couple of notes between each D" then it stops sounding again. If I play C#'' then D'' the D will come out straight away, if I play C'' and then D'' it won't.

Does anyone have any ideas what could be causing this and what I can do about it?
