Elizabeth's diagnosis sounds correct to me. Try putting a cigarette paper under the pad and then press it down onto the crown gently, using light finger pressure. This will have the effect of widening the sides of the incision. Do this several times (gently, remember!) until the problem goes away. And have the pad replaced asap.
I trained myself to touch 2nd octave instead of putting the full weight of my LH into it, after it happened to me the first time.
