do you play a measured trill?

I usually do in most situations. Figure out a number of alterations per note (say 8 per quarter, or whatever) that will make the note sound "trilly". This way, you can actually practice it the normal way, starting from slow, etc. I have found this to be the most useful exercise in having reliable trill motion. After, if you want to get fancy, you can easily do so.

When doing the exercise, really make sure that the notes are even - don't hold the first note too long (as in dramatic cadenza ending trills).
