Occasionally that transition gets out of sync for me as well. First I try to determine whether the problem is a sluggish left hand middle finger lifting, or a sluggish right hand index finger depressing, by listening to the pitch (it will be either B or B flat) of the unwanted intermediate note. Then I work slowly with that information: if the problem is an unwanted intermediate B flat, then I play A, B, C repeatedly, gradually increasing the speed; and if the problem is an unwanted B, then I play A, B flat, C repeatedly. I then work A to C slowly, not moving to the C until I have thought about (and consciously feel) the sluggish finger. Then increase the speed, still trying to think about the reluctant finger. For whatever reason, this works for me.
