My suggestions would be the same as I would give to a "pure" beginner. Find a qualified teacher, as it is easy to learn bad habits without proper guidance. Start slow, and do the "hard work" that you are now mature enough to handle. Your years of experience have probably given you the gift of patience (that most of us lack in our younger years!).
The biggest bit of advice is to avoid tiring the embouchure and forcing it into an unacceptable posture. When you get tired, stop. Come back later, and continue. That way, you will build endurance and reinforce good habits, rather than learning bad ones.
One of my early teachers used to say: "Leave time to do it right, or make time to do it over."
