I think sightreading is one of the hardest things to learn, but then, that might be my own learning disability. There is really no substitute for experience. I think really good sightreaders are those who have seen it all before. They look at a few measures of an odd collection of notes and rests and think, "ah, mambo rhythm, I've done that before" and then go on to play it with aplomb. Or, some arpeggio with a bunch of accidentals, and they recognize it as the relative minor subdominant* and poof, it is done.
(*If this doesn't make sense it's because I don't know what I'm talking about, since I'm not one of those people....)
