I think your physiotherapist knows what she is talking about.
When I concentrate on exhaling I think I do the correct breathing, but if I think about inhaling so much air as possible, I am quite sure that I use my shoulders to help me.
For me a harder reed demands more breathe with the diaphragm and less mouth support and the opposite for a softer reed, so I guess the correct breathing and a harder reed will probably help you get rid of your problem.
