I have very small hands, too, Ashley -- can't reach an octave on the piano. I've had no trouble adjusting to the English Horn. It just feels like a big oboe. It does not require all that much more stretch -- at least, my Fox doesn't.
It is heavier than the oboe -- that's the most significant difference, in my opinion. If I had to play it for a long time, I would want a floor peg or a neck strap. (I got a fhred, the one that is supposedly designed for the English Horn, but for some reason, it's too long for my instrument, even in its closed position.)
I do use a Ridenour Thumb Saddle on the English Horn thumb rest, as I also do on my oboe.
