From the description above, I would think this is a Lelandais mouthpiece. Don't know much about them, but I believe they are highly regarded.
And I also agree that this is a single-reed oboe mouthpiece.
You might check on the Clarinet Board -- either post the query, or use the search function with the word "Lelandais".
