My understanding is that the "45 degree" figure is a rule of thumb, but that like most rules of thumb, it's subject to individual variations. If you tilt it up too much, you're bringing your top lip too much to bear on the top reed, and if you crouch over it, you're putting too much pressure on the bottom reed. So you want to strike a happy medium, so that the reed is being influenced according to what you want it to do for pitch and tone, etc.
Nobody's sitting there with a protractor measuring your angle. :D
Also, people's bodies vary, so the girl with the oboe bell down around her knees may just be shaped so that in order to have the reed at the correct angle, she needs to have the oboe like that. Or maybe she's just got poor posture.
