i'm voting for old tired reeds, this seems to be the most likely common reason for blowing consistently sharp, reeds generally blow sharper with age
for replacement and turn-over purposes i estimate 10-12 playing hours for a medium to medium hard reed, but at whatever point in time any reed makes me really work keeping F# and G down to pitch (my oboe's most readily # notes) i retire them, and about six weeks or so later, i try salvaging any for use as quiet, mellow home practice reeds
when you get your new reeds, after blowing in, they're settled down and generally good, test your oboe using *neutral embouchure* start at A440 but don't adjust embouchure for any note, and identify the notes that tend to play flat or sharp, do that with several good reeds over a period of time to identify a pattern for your oboe
this makes a baseline reference to help you tell when your oboe needs tweaking adjustments too, when pads and keys gradually ease up higher, notes first seem to get 'brighter' and then go #
