I had (have) problems with all aspects of rythem mentioned above. I've improved my rythm greatly by joining a jazz band. Rythms tend be rather odd in alot of jazz styles and become extremly important (Sonny Rollins could solo on one note with complicated enough rythm and make it interesting). The lack of a conductor forced me to start counting rests in my head, thinking hard about every rythm on the page and we sight read alot of of music, sometimes in performance. Even more daring than the jazz band was the jazz combo I played with for one gig, sight reading jazz solos in performance was terrifying but by the end of the night I got pretty good at it.

Also, get out your metronome, plug it into big speakers/amps and learn to despise it so much that you play with perfect rythm to avoid having to use it again...ever.
