I don't know if this is at all helpful, but I am primarily a clarinetist and I get the same symptom that I think you are talking about. It sounds to me like you are getting a soft palate leak. When the muscles in your throat get tired, your soft palate gives way and air leaks out through your nose (? does this sound right?) Unfortunately, I can't really give any advice as to how to fix it. I've heard it is common among players of "high pressure" instruments like oboe and clarinet. What works best for me is to be conscious of the tension in my throat, and to stop when I have that soft palate problem. If I keep playing, I irritate the back of my throat and it takes longer to go away.
