I suppose it is a good idea to read through a score first of all, noting obvious structural things -- like, for example, a series of notes on the final beat in a measure as probably belonging to the following, rather than preceeding phrase.
But there are some things -- a lot of things -- that just have to be done "on the hoof", IMO.
I guess I'm always playing one step ahead of my current comfort level. I find that it takes me a few days to get the notes under my fingers, and until I achieve that, you might as well be talking to a wall as far as musical nuance goes.
Now, on easier music, the situation is different -- I DO attempt "instant musicality".
