Look up a Surgical Supply store or a Medical Equipment store in the yellow pages. Go there and get a straight hemostat. Use it to grab the bottom of the swab from below and pull it out the bottom of the joint.
Better yet, go to a musical instrument repair shop and have the technician pull it out. It's worth the lecture.
