removing/replacing octave inserts

I have a question for those of you who do repair work. How do you remove and replace the octave inserts on oboes? I see that there is a tool available for this but I've never tried it myself and have never watched someone do it.

How are they held in place, are they threaded?
Do you usually end up having to replace the pad if you remove/clean/replace the octave insert?
