The exercises which Drew suggested have always helped me. It is particularly helpful, as Drew suggested, to discern which finger it is which is slower or faster when making the transition. One additional thing which I do when reading music with the A to C sequence is to draw a vertical line between the notes with pencil. Why that works for me is inexplicable; maybe it somehow focuses my subconscious mind on the desired result.
