Thanks all for the helpful posts, especially Chris. Crikey, did you slit a wrist onto the page?! It's like you bleed information! I am still trying to digest.
Turns out that the side octave was slightly blocked - good call J. A is now possible, although slightly flat - think that is me. B is also there and not quite as flat as the A (relatively speaking of course) but it is very quiet. I think this is a breath support and embouchure strength issue.

I'm waiting to hear back from a teacher, in the meantime I'm learning all the bad habits I can!
