 I'd second the recommendation of Roberto's.  I know Kristin Bertrand there and she does fantastic work on par with many of the greats.  Definitely worth a trip to see her.

 Susan Eberenz, vintageflutes@aol.com (917) 861-6234, has done excellent work for me.  She's a flute specialist who apprenticed at Haynes, but she also works on clarinets.  She now lives and works in northern New Jersey, near NYC.

 I third the recommendation of Roberto's. 
Kristin Bertrand is a clarinetist who does great work. 
And ask to see the case humidifier she invented.
