I assume it's probably a ring key conservatoire system with LH C# - no special name for this type as extra keys can be added either as standard or (most likely with this type of LH C#) to a customer's specific request.

And the LH C# is very a useful addition which I wouldn't mind having fitted.

A LH C# lever can be fitted to any oboe, but it will be expensive due to the amount of work needed doing and depending if it's a long lever key, or part of the feather keys just like the LH cluster as your Cabart has (and a LH F lever can be fitted over the top of these if wanted), whereas a LH F is much easier to fit to most oboes.

More usual was to have the LH F as the extra feather head and the LH C# overlapping them all (as a standard LH F would do now). See Leon Goosens' book for photos as he played an early 1900's thumbplate system Loree with the LH F and C# arranged in this manner.

Nowadays if both are fitted to the same instrument, the LH F and C# are both lever keys with the touchpieces next to each other, the LH F being on the outside with the C# key arm running down between the feather barrel and the RH fingerplates.
