Two things:
1. Blowing on a reed
Just walk round the house / drive in your car blowing on a reed. It drips, so you need a small handkerchief. Don't support the reed with your hand - use your lips to hold and to blow (Don't bother about breathing in. Who needs to do that, anyway? ) Blow loud and soft, high and low, staccato and legato.
2. Rubber exercise balls
I have small rubber exercise balls in different strengths (I got them in a store that specialized in rehab equipment). Just squeezing them occasionally for a few days or so will bring back the muscle-tone to your forearms and the strength to your fingers.
You'll be surprised how quickly you'll get back to form!
