This has happened to me in the past and I've always found it was for either:
1. tip too thin (really too thin)
2. tip not extremely thin, but too much difference with the heart
3. step between heart and tip too sharp.
There could also be a leak in the reed. In such cases, I have not found fishkin, cellophane etc. to really help. I have re-tied some reeds, and it has worked, but other times no: sometimes the leak just won't close due to gouge inconsistencies or if the cane is just too clearly molded to the staple from the previous tie.
#2 is the easiest to fix by just reducing the front of the heart (sides, middle, overall) to ensure a more gradual blend.
If necessary (and if the reed is long enough), you can "back-up" the blend and chop the tip as I explain on my web-site.
#1 & 3 can be fixed the same way, but its much more difficult.
