The alternate C# is not common at all, although Rigoutat offers it as an option according to their website (http://www.rigoutat.fr/pro/hautbois/). "Ut# gauche" is the left-hand c#. On one of Holliger's album covers you can see that the oboe he was playing included it.
I don't know if Loree offers something similar, but I can't imagine that it's worth the money. I wouldn't want to get used to having something so much out of the mainstream.
WRT the third octave key. I like having mine but I can count on the fingers of one hand (with a few fingers left over) the number of times I've used it in performance. The third octave key for English horn is another matter and I use that much more often.
