The meaning might be used differently in different geographic areas.
Around here, it basically means anyone who acts superior to those around them, or expects outlandishly out-of-normal attention, support, compensation, perks, or service. Furthermore, the person usually acts as if others have no other purpose than to be servants to the person in question.
Essentially, if you look down on others...you'd be well on your way to being a Prima Donna...add contempt or outlandish demands, and you'd probably have reached the status.
That's how the label is used around here anyway. Oh - and to answer your last question: "How would someone be able to tell if they themselves were one?" - The person wouldn't know they were a prima donna, or admit to it. See also: Narcissism.
