Info on Terry Horlick's DIY case?

I'd like to try making a simple clarinet case and after searching the forum, I found many references to Terry Horlick's instructions for doing this.
Unfortunately, all the links seem to be dead.
Does anyone have a copy of the instructions, photos etc?

The instructions are at http://test.woodwind.org/clarinet/BBoard/read.html?f=1&i=97451&t=97451. They work for any briefcase. Steve Fox, for example, uses what I think is this method to make cases for his reproductions of old clarinets.
