I'm not sure why I'm responding since it took many practice sessions of my own to figure these out and anyone who cares enough should do it on their own, but here goes...
D4 is simply fingered like A4 (like a high E) and lipped up again. the E flat above this is fingered like a high F sharp with the thumb (and reg. key), left hand middle finger but you must add the first finger of the right hand.
