Practice, practice, practice. After 20 years it's going to take time to regain your skills and build up your breath support and embochure. As for reeds, start with a softer (lower number) reed and work your way up. You might consider joining a community band or other such group. You can get support from the other players and have fun at the same time.

I went on holiday and didn't practise clarinet.. Now I can only blow 3 half reeds.. hehe....
