I haven't had back problems but I did suffer from tendonitis in my right arm that caused me to have to use a neck strap. The tendonitis lasted about 2 years including about 6 months of using a neck strap while playing the oboe. After my arm healed (mostly because of a cortisone injection in and around the tendon), I slowly learned to stop using the neckstrap.
If you do decide to use a neckstrap, I suggest getting one of those bungy-cord neckstraps that BG makes. That's what I use with my english horn and it gives me just enough assist so that not all of the weight is removed from my thumb.
