Hello...
I used to have that problem myself. Sometimes it tends to come from a reed that is played out, too easy and is very closed, making you work harder than you need too. My reeds are easy, but are not so easy that I am clamping the reed to make sure it speaks... It is also a result of playing too long without giving yourself a break. If you are not used to playing 5 hours in a day, you might want to consider scheduling certain hours of practice to build up your soft palette, but remember to schedule in a 10 minute break as well.
