On most full professional model English horns (with split ring D) it is possible to re-engineer the key-work slightly to accommodate a player with small hands.

Some suggestions:

Left Hand:
Re-engineer the B key split plate so that the split between the plates is slightly lower position
Re-englineer the G touch so it has an upward extension. Sometimes just soldering on a round extension plate.
Left hand F touch can be extended
Left B and Eb touch can be extended, G# touch can be bent upwards

Right hand:
A long extension plate can be added to the F# touch
The C-D trill lever can be moved down
The right pinky key cluster touches (C, C#, and Eb) can be bent upwards about 1/8 inch.
An adjustable thumb-rest can be added. Usually I recommend a Selmer adjustable thumb-rest designed for bass clarinet. The part is about $45 in silver plate. This often is a huge help in combination with key work revisions.
Marvin Kranz (St. Louis Woodwind & Brass) is usually the oboe specialist I recommend to accomplish keywork revisions.
The Kreul English horns (Kreul, Kreul/Gordet, Kreul/Mirafone, Kreul/Lucerne,
Kreul/Genet) usually best left for players with large hands- not good candidates for re-engineering.
The "pre-war" Loree English horns overall seem to be good for players with small hands.
