I retie reeds occasionally, but if they're shaped badly or too warped, there's nothing much you can do about it and I just throw it away. But as you mentioned, if there's too much flare at the base near the string, I'll retie.
At my university, my professor insists that the freshman buy reeds (10 at a time) from stuart dunkel (she thinks making them go through the process of ordering, sending a check, and waiting a month and a half is a "learning" experience for them and therefore forbids me to sell them to them directly.) Amongst the many problems I consistently see in the reeds however are that they are frequently overtied to 48 or even 49, and I often retie them and rescrape them for the freshman. This is another instance I retie.
I don't understand #3 on your list. #5 I would think a lost cause as well since when you initially tie a reed on a staple and leave it to dry, it warps. a lot. And if you've tied it crooked, it's probably warped too much one way or the other.
Anyways, these are just my input and are very debatable.
