 Rick Sayre mouthpiece model

I have a Rick Sayre mouthpiece I am wanting to sell but I can't find any information on the model. The only number stamped on the mouthpiece is "495." I bought the mouthpiece many years ago from him when I lived in Chicago (probably early 90s) and it is stamped with the fish symbol. I used to play V12 4 reeds on it, but I would like to have more to describe it when I put it up for sale.

Does anyone know what model this might be?
