Interesting the dutch doctor distinguishes between a late rash and an early scarlet fever rash. Every source I consulted states that scarlet fever is strep throat with a rash. No other kind of rash is mentioned as associated with strep A.
In the final chapter of his "Mozart", Wolfgang Hildesheimer does a round up of then (1977) theories about the death of WA Mozart. Very interesting, if a trifle prolix. Among them is mentioned in the notes, a hypothesis advanced in a german medical journal (1967) that the cause of death was possibly "nephrosclerosis". I guess we'll never know for sure, due to lack of reliable evidence, although Salieri is pretty much exonerated these days.
