Does anyone have a suggestion for fingering a trill from A four lines and a space above the staff to a whole step above?

Since you post on the clarinet mailing list, I assume you are asking about the clarinet.
Play the A6 with the standard fingering:
TR o x x / o o o F#/C#
and then trill to B6 using:
TR + A key o x x / x x x F#/C#
For a slightly sharper B6 use:
TR + A key o x x / x x o F#/C#
