Well, I'm a bottom-swabber -- that is to say, I swab from bell to top. Just works better for me. Once, twice -- it depends on the time available and the perceived need. But the practice of leaving the swab in the instrument for a longer-than-usual period tends to make the issue of "how many times" moot.
My swab (available at Forrests -- haven't seen it elsewhere) is VERY long -- the silk part alone extends past the first joint and half-way or more down the second -- and is also wider than any other oboe swab I have seen. And no, it does not get caught.
Since I drop the swab in from the bell (being careful not to turn the oboe over on its keys before the swab gets there), I just leave it partway-extracted out the top joint (enough that the silk shows) for whatever time is available. I do think it tends to soak up more moisture with this prolonged exposure than it would in the normal quick pass or two.
