If everything 'checks out' with your instrument, your embouchure, and your reeds, then you might have a problem with your facial/jaw muscles.
You can check by closing your lips tightly and blowing into your cheeks, looking in the mirror and making sure both sides are even.
If your throat 'leaks' while playing, i.e. lets air escape either into your vocal chords or your nose, this will probably improve (decrease) with playing. If it doesn't, do consider having a medical check. Or have someone who knows you well check for symmetry when you 'scrunch up' your face, blink, etc.
Lots of responders are making excellent suggestions. I hope you find the 'aha!' moment and surmount the hurdle.
