Does anyone know of an alternate fingering for a tremolo between the throat A and 2nd register D?

Use the RH "E" side key, You must trill very fast, this D note will not sustain for long, and will be very "thin", I dont know an alternative though.

Hold the right hand notes down while you play A. I don't think there's an easy way to play this fast.

This might work (it does for tremolos from A to B, C and C#):
Play the D and, without uncovering the left index finger hole, open the throat A key. Then tremolo with your right index finger.
