When adjusting a reed with reed rush should the reed rush be held vertically or horizontally in relation to the reed?

Perpendicular, that is the long dimension of the reed rush perpendicular to the long dimension of the reed.

I don't think it matters. Hold it the way that's most comfortable and gives you the most control.
