If you want to go directly to Yamaha, I'd get in touch with their Los Angeles Atelier location: (714) 522-9011. Jeff Peterson at Yamaha does amazing work. I think he's the head of the entire Atelier now so I don't know if he's on the workbench as much, but certainly they can provide some recommendations if they aren't available.
I had my first-gen CSG set overhauled by Lohff & Pfeiffer with their Studio Setup. They tour to the US annually, so you can probably catch them sometime if/when they are on the west coast, their site is www.clarinet.dk.
My second-gen CSG set came from a colleague who originally acquired them through the Yamaha NYC Atelier where Tomoji Hirakata did the setup. Wonderful clarinets, now in the hands of one of my former students.
