 How worried should I be about this wood damage (picture)?

That's just where the end grain surrounding the pillar hole has been broken off, so hardly anything to worry about as it's just cosmetic.

Very unlikely but possible. Could even be from tightening the post rather than over-tightening. Possibly from drilling and/or tapping the hole originally. Or something else.

Pillars/posts can only be fitted in one way so the rod screw holes line up with each other, so overtightening them after they've been drilled and the key fitted isn't likely.

Maybe it was done when the pillar holes were originally drilled (if the cutter tore the grain up) or when the pillars were originally fitted before the holes for the rod screw were drilled and the pillars faced off, but in any case it's not going to be a source of worry considering how long ago it was done.
