Both index fingers (plus LH thumb on the hole & register key)

Add the 2nd finger and Ab/Eb key on many clarinets for tuning to farabout's fingering. The Ab/Eb key should already be down.

Depending what make/model clarinet you have, you don't have to use the Ab/Eb key for altissimo D. You probably do on Buffets though as they tend to sag up top compared to Selmers.

There are 19 fingerings for that G in Ridenour's "Clarinet Fingerings" book
